Parents Night Out Agreement

A COMPLETED AND SIGNED AGREEMENT AND WAIVER IS REQUIRED TO ATTEND PARENTS NIGHT OUT ... Sorry, No Exceptions

In consideration of being allowed to enter the play area and/or participate in Parents Night Out at Kids Wonder the undersigned, on his or her behalf, and on the behalf of the participants(s) identified below, acknowledges, and agrees to the following conditions:

I represent that I am the parent or legal guardian of the named participant(s) to execute this agreement on their behalf.

I agree that the participant(s) named below shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in Parents Night Out at Kids Wonder and that violation of rules may result in guardian being called to pick up child(ren) with no monetary reimbursement.

I authorize Kids Wonder staff to call 911 if an emergency were to occur. I understand that there are dangers and risks associated with activities, play, and exercise equipment at Kids Wonder Indoor Play Center. I understand that an inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. I knowingly and freely agree to assume all risk of illness or personal injury, including the potential for paralysis and death. On behalf of myself and the named participant(s), and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release and hold harmless, True Play Entertainment, Inc. DBA Kids Wonder, their affiliates, officers, members, agents, employees, other participants and sponsoring agencies from and against any and all claims, injuries, theft, property damage, liabilities or any kind of action arising out of or related to participation in any and all Kids Wonder programs, activities, parties, and the use of the play and exercise equipment.

Please do not bring your child if they are sick. Temperatures may be taken at check-in. We can not admit children with a fever or who are otherwise sick. If your child becomes ill during the program, they will need to be picked up.

All children will need to be signed in and out by {{billing_first_name}} {{billing_last_name}} or an approved parent or guardian as specified at sign-up. A photo ID will be required for pick-up.

We do not allow parents into the play area during parents night out. You will be required to stay in the lobby during drop off and pick up.

I acknowledge that photographs and/or video may be taken of my child(ren) and I hereby grant permission to True Play Entertainment, Inc. DBA Kids Wonder to use such photographs and/or video in publications, news releases, online, and in other communications related to Kids Wonder.

I am fully aware, understand, and agree to these terms and will abide by the rules listed below.
• No outside food or beverages on the premises unless coordinated in advance to meet dietary restrictions/requirements.
• No gum.
• Socks must be worn by all children (no bare feet and no outside shoes in the facility. OSHA requires employees to wear shoes, which are always kept at Kids Wonder).
• No sharp objects, including weapons or anything that could be used as a weapon.
• Children ages 3-10 years only allowed on inflatables. No adults allowed on inflatable's (NO EXCEPTIONS).
• All children must be fully potty trained.
• Healthy kids only…please!
• Play safe and play kind.
• I agree to respect and uphold all of Kids Wonder's written and verbal policies and procedures.

Parents night out starts at 5:00 pm and pick up time is 8:00 pm. Please do not be late to pick up your child. A $15 fee will be assessed if your child(ren) isn't picked up by 8:15 pm with an additional $20 for every 15 minutes thereafter. Please contact Kids Wonder to notify us that you will be late. Kids Wonder may contact you, your emergency contacts, or authorities for excessive delays.
